Student Health Records

All students (full-time, part-time, non-degree, exchange, auditing class) are required to have a completed Comprehensive New Student Health Form on file in the Student Health Center prior to your arrival on campus. These forms are only required upon admission and do not need to be submitted each year. Failure to do so may result in delays on move-in day and will preclude students from class registration.

Completed health records include the following:

  • Page 1 demographics and emergency contacts
  • Page 2 self-reported mental health history
  • Page 3 self-reported health history
  • Page 4 tb risk assessment
    Please note that a ‘yes’ answer to questions 1-7 will require a tb test (contact your family doctor or a local urgent care center to schedule an appointment)
  • Page 5 immunization dates
    Vaccine records may be obtained from your family doctor, high school, prior college/university, or the Department of Health. If vaccine dates are not available, serological titers (blood work) may also be sent as proof.

The following immunizations are required for all full-time students or a Statement of Exemption from Immunization must be completed.

  • MMR (measles, mumps, rubella); 2 doses
  • Tdap/Tetanus; given within the last 10 years
  • Hepatitis B; 3 doses
  • Varicella; 2 doses or provide date/age that student had chicken pox disease
  • Meningitis (ACYW135); must be 1 dose given after age 16

The following immunizations are required for all part-time students (less than 9 credits) or a Statement of Exemption from Immunization must be completed.

  • MMR (measles, mumps, rubella); 2 doses
  • Tdap/Tetanus; given within the last 10 years
  • Varicella; 2 doses or provide date/age that student had chicken pox disease

Students going on to become full-time will be required to provide documentation for the following:

  • Hepatitis B; 3 doses
  • Meningitis (ACYW135); must be 1 dose given after age 16

All vaccines must be given according to the CDC immunization schedule

All immunization requirements/recommendations are in accordance with the Pennsylvania Department of Health, The American Academy of Pediatrics, The American College Health Association and the Advisory Committee on Immunization Practices.

Confidentiality

If a student is over 18 years of age all medical information is, by law, confidential. No information will be given to parents, friends, college administrators or professors without consent from the student.
