Online Reserve Request Form

This form is to be used to request reserves for library-owned materials.

Library staff will retrieve the items listed and place them on reserve as indicated below.

* Phone

* Course Name
(ex. PHIL101)

Semester

Please keep the following in mind when filling out this form:

Please indicate exactly how you will tell the students to ask for the item, usually the way you cite it in your syllabus
Please allow 3 full class days for processing materials before you expect students to use the material
Items will be removed at the end of each semester unless otherwise indicated
