Required Background Checks for Education Students at Lycoming College

April, 2008

All enrolled students in ANY education program at Lycoming College must complete the following background checks PRIOR TO THE START OF THE SCHOOL YEAR (INCLUDING FRESHMEN). You will not receive a field placement until copies of your yearly up-dated clearances are on file in the Education Department Office (D207/AC).

Required Background Checks: Must be renewed yearly

1/ Pennsylvania State Criminal History Records - Act 34 or SP4-164 - form available in Ed. Dept. office

• Fee: $10.00 money order payable to the Commonwealth of Pennsylvania No personal checks accepted!
• This form may be done online at: https://epatch.state.pa.us/Home.jsp

2/ Federal (FBI) Criminal History

• Fee: $40.00 payable to Cogent Systems
• Registration information can be found at: www.pa.cogentid.com
• There are two local print sites- check the website for details

3/Child Abuse Report - Act 151 or CY113- form available in Ed. Dept. Office

• Fee: $10.00 money order payable to The Dept. of Public Welfare. No personal checks accepted!

4/ Tuberculosis test:

• You must obtain a TB test from your private dr.
• No form is provided - test information and results must be presented on your doctor's letterhead.
• A limited number of TB tests are available through the Lycoming College Health Services office. Please call them for further information - 321-4052

When you obtain the clearances/TB test give please copies to the Ed. Dept. and keep the originals in a safe place. They should be sent to: Barb Horn, Lycoming College Dept. of Education, Box 22, 700 College Place, Williamsport, PA 17701.
